Community Support Service

  The Almond Villas Group offers a comprehensive aftercare service for service users' who wish to remain in the locality. The aftercare team begin working with individuals once they are ready to start looking for their own accommodation; following them through the moving process and into their own home. Ongoing support is then tailored to meet individual need.

Ongoing support

  All service users are involved in the development of an individual support plan prior to moving which is then reviewed with them on an ongoing basis. Support plans focus on increasing and developing service users’ independence.

 Community support is flexible; support is arranged in order to accommodate service users other activities including college attendance and voluntary work as required. Service user preferences for days and times of visits are also met where possible.

 Service Users receive support from designated members of our community staff enabling them to develop trusting working relationships. All community staff receive comprehensive training to enable them to meet service user needs.

Support Available

  Support is available with all areas of independent living and includes the following:

• Managing finances including benefit applications and bill payments.
• Support with domestic tasks.
• Prompts and encouragement regarding self-care.
• Managing medication.
• Attending health and professional appointments.
• Shopping and cooking.
• Applying for college courses, voluntary work or paid work.
• Accessing community facilities, including leisure facilities and social groups
