Care Plans

Almond Villas - providing mental health care and rehabilitation for young adults with mental health problems.

Holistic care plans are formulated in conjunction with the individual service user. During weekly 1:1 keyworker sessions all aspects of life are discussed and goals are identified.

A step-by-step plan to achieve these goals is agreed and implemented with support allocated as required. Full involvement of the service user empowers them to achieve autonomy within their daily lives.

Daily Routines

In order to promote a feeling of belonging and develop a sense of responsibility, residents are encouraged to participate in the daily cleaning of communal areas of their flat. Support is allocated at a level which is appropriate to the individual and gradually reduced at a pace which is indicated by the development of individual skills in this area.

Service users have a responsibility to keep their personal space clean and tidy and are supported by their keyworker in this task, thereby establishing routines and developing skills which are required for independent living

Mental Health

Service users are facilitated to develop awareness of their mental illness and gain insight into how this impacts on their daily lives. They are given support and guidance to develop strategies to manage their mental health, thereby limiting the effects of their illness and enabling them to live integrated lives within the wider community.

Medication

As medication is a fundamental factor in the mental well-being of our service users, they receive support with all aspect of their individual prescribed regime. Throughout the rehabilitation process they are educated as to the importance of their medication, its benefits and potential side effects.

The administration of medication is detailed in the care plan. Full support and administration on a daily basis is ofered until service users move gradually onto a complete self-managing regime .
